The Change Name transaction allows a user to change or correct a customer's name, customer type, sex, date of birth, weight, height, color of eyes, and/or color of hair.
Please refer to the Driver's License Guide - Proof of Name Change section, as needed.
A customer may start the change name process on the DMV website. After the request has been made, the customer will have 14 days to come to a CSC to complete the change name process with the appropriate documentation.
To process a Change Name transaction
If not already open, Open the Customer Console using the customer's number or name.

When the customer comes into the CSC to complete the change name process, they should have with them •The legal document justifying the name change (Marriage Certificate, Divorce Decreed or court order) •Their valid credential •The DMV web generated email explaining the next steps to the customer.
Once the customer arrives at the CSC, Open the Customer Console using the customer's number or name. The Alerts grid will display the pending CHGNAM transaction. 1.Click the The Change Name transaction form opens with the customer information populated. The DL1P DRIVER'S LICENSE AND IDENTIFICATION CARD APPLICATION form prints with the customer's information populated and the answers from the web process. The following message displays.
2.Click OK to continue the Change Name transaction. 3.Make changes or corrections to the customer's name in the New Name Information section and any other information that needs changing or correcting. 4.The customer will need to complete the Motor/Voter section at the top of the DL1P form, review the pre-printed information, then sign and date the DL1P form. 5.When done, click the Process 6.Click the Close 7.Click the Close |
1.Make changes or corrections to a customer's date of birth or Sex in the top section of the form.
2.Make changes or corrections to a customer's weight, height, color of eyes, or color of hair in the Customer Information section.
Under the Motor Carrier Information section for IFTA customers:
Click the "Yes" radio button to indicate if the customer is a non-purge customer
Enter the DOT number and Payment Type in the corresponding fields
Click the "No" radio button if the IFTA customer may be purged
Note: The radio buttons will be active for the IRP In-State work center only.
3.Make changes or corrections to a customer's name in the New Name Information section.
4.When done, click the Process button. The Manager Name Change Authorization window will appear. Your manager will have to authorize the change name transaction then click OK to continue. The Name Change Effective Date will populate with the current date and is protected.
5.The Transaction Details dialog appears with a summary of the transaction.
6.Click the Close button to close the Transaction Details dialog.
7.Click the Close button to complete the transaction.
