Use the Emergency Contact Application-Print transaction to print the Emergency Contact Application (DL-569). mySelect will populate the applicant's information on the form.
To print an Emergency Contact Application
1.If not already open, Open the Customer Console using the customer's number or name.

1.Click the Transaction Menu button in the Main Menu Bar. 2.Navigate to Customer Transactions>Customer>Emergency Contact Application-Print(EMCPRT) and click it. The Emergency Contact Application-Print screen opens with the customer information populated. 3.Continue with the steps below. |

2.Click the Process button to print the Emergency Contact Application (DL-569) form and have the customer complete the form with any changes or deletions needed.
3.The Transaction Details dialog appears with a summary of the transaction and the Transaction Processed message.
4.Click the Close button to close the Transaction Details dialog.
5.Click the Close button to complete the transaction.
