Hazmat Background Check Application

Prior to processing a HAZMAT endorsement, a customer must undergo a background check.

To process a HAZMAT Background Check application

1.If not already open, Open the Customer Console using the customer's number or name.

    If the customer has had a HAZMAT Background Check, you may see one of the following alerts:    

HAZMAT

HAZMAT Background Check Expired on mm/dd/yyyy

HAZMAT

HAZMAT Background Check Expires on mm/dd/yyyy

       

1.Clicking the Button_GreenRightArrow button will launch the Hazmat Background Check Application screen.

2.Continue with the steps below.

Transaction Menu

1.Click the Transaction Menu button in the Main Menu Bar.

2.Navigate to Customer Transactions>Customer>Hazmat Background Check Application(HAZBCK) and click it. The Hazmat Background Check Application Inquire form opens.

3.Click the Inquire Button_Inquire button to open the Hazmat Background Check Application transaction form.

4.Continue with the steps below.

Command Line

1.  Enter HAZBCK on the Direct command line, then press the Enter key.

2.  The Hazmat Background Check Application transaction form opens.

3.  Continue with the steps below.

 

2.Enter the required information as indicated by the Button_RequiredField icon and any other information as necessary.

3.Enter the photo document being presented as proof of legal presence.

a.In the Legal Presence section, click the Button_Globe icon to the right of the Hazmat Photo Document field.  The Legal Presence Code Selection dialog appears displaying a list of acceptable documentation.

b.Click the Button_Select button next to the documentation type the Customer is presenting.  The dialog closes and the Legal Presence code and description appear in the appropriate fields.

Depending on the primary document entered, you may be required to enter a secondary proof of legal presence.

If you select TW for Transportation Worker Identification Credential (TWIC), the TWIC Expiration Date and Authorization Number fields must be filled in.
 
Note:  If the TWIC expiration date is within the current year, a secondary document is required.

7.If a secondary document is required:

a.Click the Button_Globe icon to the right of the Hazmat Proof of Citizenship/LPR field.  The Legal Presence Code Selection dialog appears again displaying a list of acceptable documentation.

b.Click the Button_Select button next to the documentation type the Customer is presenting.  The dialog closes and the Legal Presence code and description appear in the appropriate fields.

8.Click the Process Button_Process button.  The transaction is processed.
