Name

Data entered into the Name field must meet the following requirements:

If Individual, enter the last name, first name, middle name.

Example: LAST,FIRST,MIDDLE

•There must be two commas in the name field, if the Customer Type is (I) Individual.

•There must not be any spaces between the names and the comma(s).

•Suffixes, such as, JR, SR, II, III, 2ND, etc., are considered part of the last name.

Example: LAST JR,FIRST,MIDDLE

•Hyphenated names are acceptable as part of the name.

Example: LAST-MAIDEN JR,FIRST,MIDDLE

If company, enter the full company name.

Example: UNITED PARCEL SERVICE

•Two commas are not required.
