1. Under the Operating Authority and CLPs / Classification (UCR) information bar, when clicking the Button_PlusSign to display additional information of an authority type or UCR, the labels have been changed from  Registration Year 1 through Registration Year 3 to UCR Registration Year 1 through UCR Registration Year 3.

2. Under the Operating Authority and CLPs / Classification (UCR) information bar, when clicking the Button_PlusSign to display additional information of an authority type (excluding UCR) populate these fields with N/A.  (These registration fields pertain to UCR only.)

3.  Move Message field from Customer Address Information section to the Operating Authority and CLPs / Classification (UCR) information bar section. Place the Message field under the UCR Registration Year 3 field. (This message information pertains to UCR registration being processed through Indiana.)

4.  Change label on Customer Address Information bar to Customer Primary Address Information.

5.  At the top of the screen, add word Federal in front of  Motor Carrier (MC) Number to specify this is the Federal Motor Carrier Number as opposed to the DMV Motor Carrier number.

1.  Collapse sections with column labels only (if there is no data populating the columns)

2. Under Additional Customer Number Information bar, change Liquidated Damages (LD) Number to Citation Tracking (CTS) Number.

eForms Button
